Troubleshooting
Most reports about Tangible Quiz turn out to be one of the behaviours below. Each entry says what you see, and why.
Authoring
The Builder tab says to publish first
The quiz outline only exists from the post's first publish. A never-published quiz stays on a "publish first" empty state instead of showing a builder.
Publish or save the quiz on its Quiz Page tab, then return to Builder. See creating a quiz.
Editing a question changed it in another quiz
Questions live in a site-wide pool and quizzes reference them. There is no copy: one question edited is that question edited everywhere, delivered when each quiz (or the shared section carrying the question) next publishes.
The Associated Quizzes panel in the question editor's sidebar lists what a question is used in. Check it before editing something you did not just create. If you want a variant, duplicate the question rather than editing the shared one.
I published a question and the quiz still delivers the old version
By design: publishing a question updates the pool, not what published quizzes hand to candidates. A quiz using the question directly gains a draft and a Questions have changed banner; publish the quiz to deliver the update. A question inside a shared section delivers when the section republishes. Attempts already in progress finish on the content they started with. See publishing is what delivers question updates.
Removing a question also removed it from another quiz, or didn't
Remove on a row in the quiz builder takes the item out of that outline only. The question stays in the pool and in every other quiz.
Deleting a question outright is done from Tangible Quiz → Questions, and that does affect every quiz using it.
Maximum earnable points will not let me type a value
For automatically graded questions it is derived from the answer points rather than entered. Change the points on the answers and the total follows. Where grading criteria are in use, it syncs to the highest criterion.
I cannot find Companion Materials
It is not there. The panel (calculator, digital material, physical material) was a design mockup that never reached the save path: edits made in it were discarded on the next save without a warning. It is hidden until it has somewhere to persist to.
If you saw it in an older draft of the documentation, that documentation was describing something that never worked.
Changing the difficulty scale moved my questions
Every question must carry a difficulty, so removing a level always rehomes its questions rather than orphaning them. Removing a level that has questions asks you where they should go; anything removed without a destination, including Remove All, falls to the surviving easiest level.
Renaming a level is safe and migrates nothing. If the intent is "call this something else", rename it rather than removing and re-adding. See site settings.
Taking a quiz
A learner says they lost an attempt by just looking
They did not. Opening a quiz creates nothing: an attempt becomes real on the first autosave or save, or at submit. Someone who opened the quiz and navigated away has consumed nothing.
The Retake button is missing
The learner has no attempts left, and is in the blocked state rather than the review state. Either raise Restrict quiz retakes for everyone, or grant that one candidate another attempt, which is recorded with your name and reason.
A learner who already passed is offered a retake
The quiz has been republished since their attempt, and attempts are counted per published revision: a new revision is a new quiz to pass, so the banner offers the new version with the attempt allowance counted afresh. The learner's passed result on the old revision still stands.
The answer review doesn't show the correct answers
The quiz's Show correct answers in the answer review switch is off, under Results. The learner still sees their own answers marked right or wrong; the key itself is withheld by the server. This is the setting working, not a rendering fault.
Everyone passes this quiz
The quiz authors no way to fail: no Minimum passing threshold and no question marked Required to pass. With no criterion, a submitted attempt passes. Set a threshold, mark the questions that must be passed, or both. Only marked questions gate, so a wrong answer on an unmarked question cannot fail a candidate who clears the threshold.
A guest can read the quiz but nothing saves
That is display mode. Guests get a read-only rendering of the published quiz with no attempt behind it; only logged-in users get a real session.
If the quiz should not be readable at all, take it off the public page. The player's audience is the host page's audience, and the shortcode does not gate the page for you.
The learner submitted but sees no pass or fail
The quiz contains a question a computer cannot mark. The attempt is closed but undecided, and stays that way until a grader settles it.
My feedback message is not what the learner saw
A grader note replaces the default feedback rather than appearing alongside it. The note field shows the default as its placeholder while empty, so what you are overriding is in front of you. If the default should stand, leave the note empty. The learner reads the note in their answer review, beneath the graded question.
Grading
The grading queue page is blank
Access is decided per viewer at render time. A guest gets nothing at all, so the host page stays clean for the public. A signed-in viewer without grader access gets a card saying so, which means a genuinely blank page for a signed-in user points at the page, not permissions.
Check that the viewer is logged in, and that they either hold the grading capability or have been granted specific quizzes. With the LMS present, course mentors are granted the quizzes in the courses they actively mentor, so a mentor with no active mentees correctly sees an empty queue.
An attempt has sat in Ungraded for days
Closed is not graded. Submission closes an attempt; a verdict requires a human when the quiz has manually graded questions. Ungraded is the queue working, not a stuck job.
Nobody is assigned to grade these
The quiz has no Default scorer. Set one under Settings → Scoring on the quiz so manually graded submissions have a named owner. See quiz settings.
Course and Step show a dash
Those columns are answered by whatever plugin embeds the quiz. A quiz taken outside a course has no course or step to report, and the queue falls back to the quiz title. That is expected.
I unlocked a question and now I cannot grade it
By design. Unlocking stages the question for return, and grading is disabled on it. Before you press Save, Lock un-stages it if you clicked by mistake. After the save it is with the candidate, and grading resumes when they resubmit. The grade you already gave stays visible to them as feedback throughout.
Elsewhere
An editor cannot publish the course the quiz sits in
That is an LMS permission, not a quiz one. Editors can create, edit, and preview courses but cannot publish or delete them. See roles and permissions.
A video step will not enforce watching before completion
Also LMS-side: plain oEmbed embeds do not report playback progress, so there is nothing to measure a completion requirement against. Use Brightcove, Vimeo, or YouTube.
